It's deliberately short: just what you'll use, nothing you'll have to protect for no reason.

What's on the form

• Player details — first and last name, date of birth, gender, jersey number, and position.
• Contact — email and phone.
• Emergency contact — name, relationship, and phone.
• Licence number — for federation leagues that issue one.
• Consent & waiver — a short acknowledgement of injury risk and data use, with a signature line. For under-18s, a parent or guardian signs.

That's the whole form. It's short on purpose: every field you add is data you have to store securely and delete later, so the template sticks to what a league actually uses.

How to use it well

Set a registration deadline and collect every player's form before it — the reasoning, including two-batch registration windows, is in our player registration guide. Keep signed waivers matched to players, store the forms somewhere access-controlled rather than a shared drive, and when the season ends, delete what you no longer need. It's a consent record, so treat it like one.
