What's on the form

That's the whole form. It's short on purpose: every field you add is data you have to store securely and delete later, so the template sticks to what a league actually uses.

How to use it well

Set a registration deadline and collect every player's form before it — the reasoning, including two-batch registration windows, is in our player registration guide. Keep signed waivers matched to players, store the forms somewhere access-controlled rather than a shared drive, and when the season ends, delete what you no longer need. It's a consent record, so treat it like one.
